- Behavioral Assistance Service
- Dental Services (Adult / Child)
- Dietitian Services
- Environmental Accessibility Adaptations w/ Assessments
- Specialized Mental Health
- Support Coordination (Full / Limited / CDC+)
- Occupational Therapy
- Physical Therapy
- Residential Habilitation (Group Home - Behavior Focus and Intensive)
- Respiratory Therapy
- Speech Therapy

It is recommended, prior to contacting Mrs. Camplin, that you review the provider requirements in the Development Services Coverage and Limitations Handbook for the service(s) you are interested in becoming enrolled to provide. The Developmental Services Waiver Coverage and Limitations Handbook can be found on the APD website (http://apd.myflorida.com) under the Providers Tab (http://apd.myflorida.com/providers) and the third bullet under the heading "Medicaid Waiver Information".
