New Provider Enrollment
Becoming a Provider is a free process.
**If you would like to become a Medicaid waiver provider or have questions about this process, please contact Roberta Phifer, Vendor Enrollment Specialist at (904) 992-2426 or email firstname.lastname@example.org. Provider application packets cannot be mailed out but Roberta will email you a provider application packet upon request.

Apply for Services
To apply for services from the Agency for Persons with Disabilities, including the Medicaid Waiver program, for yourself or a family member, please leave a voice message at (904) 992-2428. Your call will be returned within 24 business hours.
Services are not based on your income but are based on the developmental disabilities covered under the Florida States 393. "Developmental Disability" is a broad term that refers to a variety of conditions that interfere with a person's ability to function in everyday activities. Chapter 393, Florida Statutes, defines developmental disabilities as spina bifida, autism, cerebral palsy, Prader-Willi syndrome and mental retardation.
