Applying for Services

To apply for services from the Agency for Persons with Disabilities, which includes waiver participation or placement in a Developmental Disabilities Center (DDC), contact the APD offic that serves your area.

Consumer Directed Care Plus (CDC +)

CDC+ is a long-term care program alternative to the Medicaid Home and Community-Based Services (HCBS) Medicaid Waiver. The program provides the opportunity for individuals to improve the quality of their lives by being empowered to make choices about the supports and services that will meet their long-term care needs and to help them reach their goals.

Emergency & Disaster Information

APD suggests developing an evacuation plan, a communication plan, and obtaining general disaster supplies. However, a person with a disability may have additional planning to do. Don't put off until tomorrow what you can plan for today!

Quality Living

Supports quality assurance and quality improvement initiatives through the Quality Management unit.
