  A publication that introduces customers to their rights under HIPAA and explains how APD protects their health information.
• Your Privacy is Important to Us - Receipt
  The receipt that customers must sign when given a copy of the "Your Privacy Is Important to Us" brochure. The brochure must be given to a client, and a signed receipt obtained, when the customer first begins receiving services from APD, and once per year after that.
